ADULT TUMBLING 757 WAIVER AND RELEASE OF LIABILITY

Please review and complete the following waiver. This waiver must be reviewed and completed in its entirety prior to event participation.

DISCLAIMER: SANELE LASANA LLC, dba ADULT TUMBLING 757, IS NOT RESPONSIBLE FORANY INJURY OR LOSS OF PROPERTY TO ANY PERSON OR ENTITY WHILE PARTICIPATING IN ANY OF ITS CLASSES, EVENTS, OR ACTIVITIES FOR ANY REASON WHATSOEVER, INCLUDING ORDINARY NEGLIGENCE ON THE PART OF SANELE LASANA LLC, dba ADULT TUMBLING 757, ITS OWNERS, OFFICIERS, EMPLOYEES, SPONSORS, INSTRUCTORS, AGENTS, REPRESENTATIVES, AND VOLUNTEERS.

 

ACKNOWLEDGEMENT: I, the above-referenced participant, am aware of, acknowledge, and accept the inherent risk of injury involved in all physical fitness activities, including aerobics, mobility training, tumbling, and strength training. Some of these injuries may include, but are not limited to, physical exertion, falls, sprains, fractures, contact with other participants, equipment-related injuries, and the potential for serious bodily harm, and death. I voluntarily choose to participate in the classes, events, or activities offered by SANELE LASANA LLC, dba ADULT TUMBLING 757, with full knowledge and acceptance of these risks.

 

As a voluntary participant in the classes, events, or activities offered by SANELE LASANA LLC, dba ADULT TUMBLING 757, I hereby agree to release, discharge, and hold harmless its owners, officers, employees, sponsors, instructors, agents, representatives, and volunteers from any and all claims, demands, actions, or causes of action arising out of or relating to any loss, damage, or injury, including death, that may be sustained by me or any property belonging to me, whether caused by the negligence of SANELE LASANA LLC, dba ADULT TUMBLING 757 or otherwise. I acknowledge and accept that it is my responsibility to maintain appropriate personal insurance coverage to protect myself in the event of any injury or loss.

 

I affirm and declare that I am physically fit and able to participate in the classes, events, or activities offered by SANELE LASANA LLC, dba ADULT TUMBLING 757. If I have any medical conditions, allergies, or limitations that may affect my participation or require special attention, I have disclosed them to SANELE LASANA LLC, dba ADULT TUMBLING 757 by providing a brief description in the space below prior to participating in any classes, events, or activities.

I affirm and declare that I am not under the influence of any drugs, alcohol, medication, or any other substance that could impair my mental or physical function while participating in the classes, events, or activities offered by SANELE LASANA LLC, dba ADULT TUMBLING 757.

 

Iaffirm and declare that I, as well as all household members, do not currently have, nor have experienced the symptoms of any communicable bacterial or viral illness or infection including, but not limited to, cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, new loss of taste or smell, and/or vomiting, WITHIN THE LAST 14 DAYS. I affirm that I, as well as all household members, do not have any pending test results for any bacterial or viral illnesses or infections. I affirm that I, as well as all household members, have not been diagnosed with any bacterial or viral illnesses or infections WITHIN THE PAST 30 DAYS. I understand that SANELE LASANA LLC, dba ADULT TUMBLING 757 cannot be held liable for any exposure to any bacterialor viral illnesses or infections caused by misinformation on this form or the health history provided by each participant.

 

I acknowledge and accept that SANELE LASANA LLC, dba ADULT TUMBLING 757 is not responsible for any of my personal belongings that may be lost, damaged, or stolen during its classes, events, or activities. I grant permission to SANELE LASANA LLC, dba ADULT TUMBLING 757 to use photographs, video recordings, or any other media taken during its classes, events, or activities for promotional, marketing,or educational purposes, without any further consent or compensation. I acknowledge that this Waiver and Release of Liability shall be binding on all my heirs, assigns, and legal representatives. By electronic signature below, I acknowledge that I have read and understand this Waiver and Release of Liability, and I agree to be bound by its terms and conditions.
