This form is for the 2023-24 school year only. Care starts on the first day of school  (Bethel) and automatically ends on the last day of school in June.

  You will need to complete a waiting list again for summer child care and also for the next school year. A new form will be provided on our website when sign-ups are available.

  ECC is closed an estimated 12 days per year and open 7am-6pm on No School Days (pre-registration required with a $50/day fee). Research shows that year-around, quality child care helps to limit life transitions and promote stability for the child.

Request Start/End Care Date

Starting child care - Registration steps will take admin a couple days, so please select a start day at least 3 business days later than today’s date so you have time to finish the enrollment steps. Your 1st month's bill will be prorated given it is not in September or June. (no proration in September or June).

We can reserve your spot for up to 2 weeks from when we email you to send the enrollment forms.

Ending child care - Complete this form by the 15th of the month and the last day of care will end on the last day of that month. The last month is not pro-rated, including June.

- Weekly schedules must be the same days throughout the month. You can message teachers via Brightwheel to make daily schedule adjustments and report absences.

- First month is prorated according to your start date (no proration in September).

- Last month is not prorated and care will automatically end in June (or August for summer camp) but you can sign-up for the next program online. See more tuition policies at the bottom of this page.

- End child care by completing this form by/before the 15th of the month to end care at the end of this month.

-Our office will review availability and email you with confirmation or follow-up questions.

NOTE: Siblings attending any ECC program? The oldest sibling will receive a 15% per month discount if both children are ages 3+. This discount is automatic for private pay and ERDC paying families (not applied to other discounts).  

  If a parent (or legal guardian) is a Lane County public or private school staff member, teacher, or administrator, please have your employer complete the form below.

  Spaces are limited and filled first come, first serve. If spots are full you can stay on our waiting list or attend at regular costs until a discounted space is available. This discount expires 8/31 of each year.

Signers/primary payers must agree to the following.

- I understand that it is my responsibility, as the primary payer, to follow all ECC tuition policies that are required. Any secondary payers will also follow these policies.

- I understand that, if the ECC administrative office does not receive the schedule change form by the 15th of the month, no billing or schedule adjustments will be made for the following month.

- I understand that ECC does not pro-rate the last month of care and I will be charged until the last day of the month, even if my child does not attend. ERDC/state pay may NOT cover the entire month's charges if I fail to ensure that my child(ren) attend the required minimum hours for my rate bracket throughout the entire month (and less than the maximum). This means I could be charged out of pocket, even if I have a $0 copay. It is my responsibility to consult ERDC's Direct Pay Unit to ensure my schedule will meet my billing needs.

- I understand that these enrollment and tuition policies can be found on the ECC website (creativecareeugene.org) and are also available upon request.

- I understand that my enrollment spot will not be saved if this form is not fully complete or if information on this form is not accurate. I will submit a new form if any of this information changes.
