This Contract is for Ad-hoc holiday care for your child.  Prior to each holiday, you will be sent a holiday request form which you will need to complete and submit if you require childcare for the upcoming holiday.  The request form is also available via a link on the Parents Page on our website.

Parent Information

I/We, hereby agree to read all the Policies and sign the policy checklist.  I/We will complete and sign all the relevant permission forms.  I/We agree that once this contract is signed, it becomes legally binding.

Please complete and submit the holiday request form at least four (4) weeks prior to the holidays when childcare is required.
