Informed Consent and Acknowledgment

I hereby give my approval for my child’s participation in all activities provided by Next Level Cosmetology Camp for Kids or Next Level Barbering for Kids during the selected camp. Our program covers important topics beyond cosmetology and barbering, including hygiene, self-care, and sexual health education to ensure students are informed about their bodies and overall well-being. By signing, I acknowledge and consent to my child learning about these essential topics in a safe, age-appropriate manner.

In exchange for my child’s participation, I assume all risks and hazards associated with camp activities and release, absolve, and hold harmless Next Level Cosmetology Camp for Kids and Next Level Barbering for Kids, along with their officers, agents, and representatives, from any liability for injuries incurred while traveling to, participating in, or returning from camp sessions.

In the event of injury, I waive all claims against Next Level Cosmetology Camp for Kids and Next Level Barbering for Kids, including staff, coaches, affiliates, sponsors, advertisers, and venue owners. I understand there is an inherent risk in all activities, and I accept responsibility for my child’s participation.

Hands on and Photo/video consent

We are excited to inform you about the upcoming Next Level Cosmetology Camp for Kids, where students will have the opportunity to enhance their skills in lashes, makeup, facials, hair braiding, and in addition to the hands-on activities, we may capture pictures and videos of the students during the camp for educational and promotional purposes. Your consent for the use of these images is crucial. Your signature grants participation in the camp activities and allows us to document their experience visually. Medical Release and Authorization As Parent and/or Guardian of the named child, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me. Permission is also granted to the Next Level Cosmetology Camp for Kids and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility. Release authorized on the dates and/or duration of the registered season. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
