Hello Honors Students, The Honors Capstone represents the culmination of an Honors student’s undergraduate experience. Capstone Mentors play a vital role in guiding students through the development, execution, and completion of independent research, creative, or applied projects. This form allows the Honors College to know that you, together with your capstone mentor, are ready to begin the Honors Capstone project. Please complete this form after you and your mentor have discussed your capstone project idea, research methodology, timeline for completion, and how your project will meet the seven Honors Capstone Learning Outcomes. Complete the Student Portion, including your signature, and then scroll to the BOTTOM of the entire form and push SUBMIT. The JotForm will then be delivered to your mentor's Inbox with a subject line of "Honors Capstone Approval Needed".

Please note: If you are working with a capstone mentor (identified above) who is not a UW faculty member, they will be asked below to complete an additional form to help the Honors College understand their expertise, mentoring experience, and the types of capstone projects that they are well-suited to supervise.
Typically Honors students identify one person with expertise related to their project to serve as their Honors Capstone mentor, however students may choose two mentors to guide their project. If you have identified an additional mentor, please write their name, title/ affiliation, and email below. Remember that capstone co-mentorship requires that YOU submit an initiation form for each mentor (and each will be required to complete an evaluation form at the conclusion of your project). Thanks!
Learning Outcome 1: Students will define a question that their project will address, responding to a problem, concern or need. (Remember that the project can be scholarly, creative, service-oriented or entrepreneurial). What is your Honors Capstone question?
Learning Outcome 2: Students will develop a plan for carrying out the project that incorporates the following elements, as appropriate to the nature of the project: relevant skills, methodologies, bodies of knowledge, and field-specific or interdisciplinary conventions. Please describe below your methodology to investigate your question and your plan for carrying out your project.
Learning Outcome 3: Students will analyze and interpret the material in their project, whether this be data, evidence, creative output, etc. What are your plans for analyzing your capstone material?
Learning Outcome 4: Students will engage in a process of sharing work with their mentor/s, receiving feedback, and incorporating feedback in the development of the project. What are your plans, and your timeline, for sharing your work with your mentor?
Learning Outcome 5: Students will create a tangible product that demonstrates critical thinking and insight in responding to the initial question. Describe your capstone product and how this will address your initial question, problem, concern or need.
Learning Outcome 6: Students will communicate their project at an Honors-approved venue through an oral presentation addressing the nature and significance of the project, as well as its implications/impact. What are your plans at this point in time for presenting your capstone project?
Learning Outcome 7: Students will reflect metacognitively on the project (ie., reflect on what they learned and how they grew from the project). I agree to reflect on the capstone process and share my process with my capstone mentor.
