Buyer is entitled to use the Academy named herein for a period that matches their payment option below.
Buyer agrees to pay the sum of the chosen membership length, the associated fees plus the number of new uniforms required and the HST.

Special Offer: 1 Student, 4 weeks of Training, Includes Offical Uniform

1 Student 3 Months=$325 or 1 Year=$1,200

Family of 2 for 3 Months=$590 or 1 Year=$2,150

Family of 3 for 3 Months=$825 or 1 Year=$2,900

Each Karate Uniform= $50

***Note all prices are subject to HST***
This agreement is subject to H.S.T.# 722403144 RT0001

This agreement is made subject to these terms:
1. If Buyer/Student breaks the payment schedule, the entire balance of the purchase price becomes immediately due, without notice.
2. Seller shall not be liable for any damages arising from personal injuries sustained by Buyer/Student in attending said Academy and using facilities and equipment and receiving instruction therein; Buyer/Student does so at their own risk.
3. Buyer/Student states that they are aware that belt promotions are not guaranteed but are up to the individual's personal progress.
4. Seller is not liable for the loss or theft or damage to, personal property of Buyer/Student.
5. Buyer/Student agrees to keep and obey all rules and regulations now in force or in the future, prescribed by the Seller and Seller reserves the right to revoke this Agreement for cause if Buyer/Student fails to keep and obey any of such rules and regulations without refund to member of any monies paid to Seller.
6. Buyer/Student states that they have witnessed a class and that all of their questions were answered in an interview prior to signing.
7. Due to sickness or absence from the city, the membership will be extended at Buyer's/Student's written request, only at such time the agreement has been completely paid in full and no other payments are outstanding.
8. I/we acknowledge that default in payment may be reported to a credit reporting agency and that such report will lower my credit rating.
9. I/we acknowledge that even if Buyer/Student does not use Seller's facility that I am responsible for payment of the full contract price as Seller's liquidated damages.
10. I/we authorize charge of any defaulted payments to the credit card named herein.
11. Written notice within 5 calendar days is needed to rescind this agreement.
12. This agreement is not transferable.
13. I have read and understand this agreement completely.
