Please complete the form below if you would like to register your child/children in the lunch program this year. Each meal comes with a side and water. All sides are nut-free options. Lunch orders must be placed by Tuesday, September 5th.
Your FACTS account will be billed on September 20th for the entire school year, or billed in two installments on September 20th and January 2nd- based on your selection below.
Should your child/children not like the food after the first week, you can cancel your order by Tuesday, September 19th. Your account will only be billed for the days that they were provided lunch. After Tuesday, September 19th, additions can be made; however, no cancellations or refunds will be allowed, except in specific situations. If you need to add or revise specific days after the first week, please email me at ascotti@sjpschool.org. Please do not fill out another order form.
Revisions can be made throughout the year to your lunch order for the following month. All revisions must be submitted to ascotti@sjpschool.org by the 25th of each month. No revisions will be accepted for June.
