MENTOR APPLICATION

Note: Please read carefully and answer all questions that are applicable. All mentors are regarded as volunteers of Reading

and Beyond. Any mentor who wishes to participate must fulfill the following requirements: completion of the mentor

application, possession of fingerprinting clearance, a TB Test, attenddesignated training sessions prior to engaging with the

youthand commit to meeting with assigned mentee 2-3hoursa month for 1 year. If your application is selected, we will

arrange a meeting to discuss the details provided in your application.
