Hey friend!
  • Hey friend!

  • Thank you for your interest in booking an appointment with Color Theory.

To ensure we are the perfect match for your hair needs, we kindly ask you to fill out this intake form. After reviewing your responses, you will receive a message or email as to whether your form was approved or declined.

If we are a good fit:  I will reach out to arrange an appointment for the consultation. These consults are a fantastic opportunity for you to explore Color Theory x The Collective, see my studio, and get to know me. During this session, we can conduct a test strand (no waiting required on your part) and map out a hair plan tailored to you. Please note as of June 1st 2026 in order to keep providing the best service for my current clientele all new client consultations will be conducted Saturday mornings from 8am-12pm.

Thank you for your trust, and I look forward to potentially serving your hair needs at Color Theory!

  • Understanding the Risks of Chemical Services:

Chemical services inherently carry certain risks due to a variety of individual and environmental factors:

    1. Personal Factors: Elements such as genetics, medications, hormones, and one's overall health can influence the structure of your hair and, consequently, the outcomes of any treatment.
    2. Prior Treatments: Previous chemical applications, even if their effects appear to have faded, remain within your hair's structure until they are cut out. These latent treatments can affect subsequent services and their results.
    3. Environmental Influences: Conditions like well water, hard water, and copper plumbing can lead to unwanted reactions with chemical treatments. The build-up of mineral deposits can unpredictably react with treatments, even if the same product has been safely used multiple times before.
    4. Allergies: It's possible to develop allergic reactions to products at any point in time, even if you've used them safely in the past.

Please note that services may be declined based on the current state or health of the hair.

    Maintaining Your Hair Post-Service:

Your hair is not just hair; it's an investment. Proper at-home care is vital for both recreating your desired style and ensuring the longevity of your color. The guarantee on color services is contingent upon the use of recommended professional products.

By affixing your signature, you acknowledge your understanding of these points. You also recognize that the longevity of your color is determined by your at-home care regimen. Should any concerns arise, it's your responsibility to inform Paige Herrick/Color Theory within 3 days of the original appointment for possible touch-up considerations. Any issues or concerns arising after 3 days fall under the client's responsibility.
