Consent for piercing release and waiver of all claims
I acknowledge by signing this waiver that I have been given the full opportunity to ask any and all questions which I might have about obtaining a piercing from Frank and Olive. All of my questions have been answered to my full and total satisfaction.
I acknowledge that I am free of communicable diseasesI acknowledge that it is not reasonably possible for the employees of Frank and Olive to determine whether I might have an allergic reaction to processes or jewellery used in my piercing and I assume such risk is possible.I acknowledge receipt for written instructions advising me of proper care of my piercing and I recognize the absolute necessity for following those instructionsI acknowledge that infection is always possible as a result of obtaining a piercing particularly in the event that I do not follow proper aftercare instructionsI acknowledge that a piercing is a change to my appearance and that no representation has been made to me as to the ability to later change or remove piercingI acknowledge that my piercing may leave a scar when removedI acknowledge that the obtaining of my piercing is my choice alone and I consent to the application of the piercing and to any actions or conduct the employees of Frank and Olive find reasonably necessary to perform the piercing procedureI have not used numbing cream on the area to be pierced in the last 12 hoursI agree to release and forever discharge and hold harmless Frank and Olive and its employees from any and all claims, damages, or legal actions arising or connected in anyway with my piercing or procedures and conduct used to apply my piercingI hearby give Frank and Olive consent to publish photos of my piercingI have read and understood all of the shops rules
By signing this form, I have had the opportunity to ask questions and those were answered to my satisfaction. I understand all the terms, my rights, as well as my obligations herein. I voluntarily give my consent and I declare that I was not induced or coerced into signing this form.

I acknowledge that I have truthfully represented myself to the employees of Frank and Olive and that I am at least 16 years of age (18 for genital piercing) and I’m competent to sign this Agreement or, if not, that my parent or legal guardian shall sign on my behalf, and that my parent or legal guardian is in complete understanding and concurrence with this agreement.

I have read this agreement, I understand it, I agree to be bound by it. I am aware that it is illegal to give false information and I hear by declare that all the information I have given is true and agree to all of the above information.
