Scheduling Retainer Policy

  • Scheduling Retainer Policy,

    Cancellation Policy, Late Arrival Policy, No Show Policy and Right to Refusal of Service.

  •       at                           UNDERSTAND and AGREES to the following policies: A 50% NON REFUNDABLE Scheduling Retainer Fee via a VALID credit card is REQUIRED and will IMMEDIATELY be charged to schedule my appointment. My Scheduling Retainer Fee WILL be used towards my service the day of my appointment, HOWEVER, should I CANCEL my appointment or I NO SHOW my appointment, my Scheduling Retainer Fee will be forfeited. I WILL be able to RESCHEDULE my appointment ONE TIME ONLY by doing so 72 hours BEFORE my appointment and my Scheduling Retainer Fee will be applied to the rescheduled appointment. If I would like to reschedule my rescheduled appointment, a 50% NON REFUNDABLE Schedule Retainer Fee will again be REQUIRED.

  • Cancellation Policy:

    I UNDERSTAND and AGREE to the following Cancellation Policy: because my appointment is held just for ME, should I NEED to cancel or reschedule my appointment, I WILL do so 72 hours BEFORE my scheduled appointment. Not giving 72 hours notice will FORFEIT my Scheduling Retainer Fee. I UNDERSTAND that I am able to RESCHEDULE my appointment ONE TIME ONLY and my ORIGINAL Scheduling Retainer Fee will be applied to the rescheduled appointment. If I would like to reschedule my rescheduled appointment, a 50% Scheduling Retainer Fee will again be REQUIRED.

     

  • Late Arrival Policy:

    I UNDERSTAND there is a 15 minute grace period if I’m running late; IF I arrive AFTER the 15 minute grace period, my appointment is CANCELLED and my scheduling retainer will be FORFEITED. A NEW scheduling retainer is REQUIRED for future appointments.

  • No Show/No Call Policy:

    I UNDERSTAND that IF I No show/No call my scheduled appointment, I WILL be charged 45% of my total treatment and will NO LONGER be able to schedule any more appointments at Beyouthiful Skin.

  • RIGHT TO REFUSAL OF SERVICE:

    I UNDERSTAND and AGREE that Grace McDonald of Beyouthiful SKin reserves the right to refuse service to anyone, at any time and for any reason.
