Register all children in your family, grades pk-12th, using this form. If your child is eligible to be in multiple sessions (eg. a 7th grader could be in Confirmation and Jr. High Youth Ministry), please select all appropriate sessions when entering registration information for that child.

Permission and Release
I/We hearby consent to my child/children participating in the Holy Trinity Faith Formation activities, those on-site and those off-site. I/We understand that some events will take place away from the parish grounds and that my/our child will be under adult supervision and all reasonable caution will be taken to prevent accidents or injuries. I/We here by release and indemnify Holy Trinity Parish, its staff and volunteers, and the Diocese of Cheyenne from any and all liability arising from claims of any kind or nature whatsoever from my/our child's/children's participation in this event, including transportation.

In the event my/our child/children were to become injured, I/we give permission for the supervising adults to seek medical care or treatment for my/our child/children. I/We understand that if my/our child/children does become sick or injured, every effort will be made to contact a parent or guardian. If Holy Trinity can not reach a parent/guardian, they will contact the emergency contact in their records.

Payment

Payment may be made online or by cash/check. No student is ever refused due to inability to pay. Please contact the office for assistance.

If paying by cash or check, please mail or drop off at the parish office. You may also place in an envelope clearly labeled "Faith Formation" and place in the collection at Mass.

If you selected to pay online, one you submit you will be redirected to the Our Sunday Visitor website that our parish uses for online giving. Select the Faith Formation Fee fund and enter the amount given above as Registration Fees Due. If you are not currently using online giving, you will need to create an account. If you do use this service, you may log in using your current account information.
