You will be taking your test in a room with several other test candidates. Please be considerate and refrain from disrupting others during their tests. Please be mindful of those around you.
After you have signed this form, we will check your photo ID to confirm your identity. ID requirements vary from test to test. Please be prepared to show us one form of photo ID (i.e. driver's license, passport, etc.) and if necessary, one other identifying document / ID including your name and signature (i.e. credit card, social security card, etc.)
In general, you cannot bring any items into the testing room besides the test materials provided to you. Some minor items may be allowed at the proctor’s discretion. Prohibited items include, but are not limited to, the following:
- Any electronic devices
- Laptops and tablets
- Bags and backpacks
- Cell phones
- watches and Fitbits, or any wearable technology
- food/drink of any kind
- brimmed hats
- headphones
- recording devices
- Sunglasses
- Wallets
- weapons of any kind
You may take a break at any time during your test, but be aware that your test may not have the option to stop your time. While on break, you will not be allowed access to , phones, other electronic devices, or any notes. Accessing prohibited items, talking to others, or leaving the building during a break are all considered misconduct and could invalidate your test.
Proctors may ask you to inspect items such as eyeglasses, jewelry, and pockets before entering the testing room.
A test proctor will provide you with erasable noteboards and markers if they are allowed for your exam. You must return these to the proctor after you complete testing. No test material may leave the testing office.
A proctor will be monitoring you at all times using video surveillance, the viewing window and occasional walkthroughs of the testing room. If you need any help or have a question unrelated to test material, you may raise your hand or exit the testing room any time.

I understand that if I fail to abide by Testing Services’ rules, ir I attempt to cheat on a test, my test could be invalidated, I could be dismissed from the test center, and the incident will be reported.
