LIABILITY WAIVER

I, the undersigned, being 18 years of age or older, (on behalf of the student named above if under 18), understand that def SOL Productions, the hosting facility and the instructors thereof are not liable for personal injuries or loss of, or damage to, personal property. I understand that participating in dance classes involves the possibility of physical injury. I therefore assume all risks involved in the participation of the def SOL Classes & or Shows whether it be I, or my participating minor child. I hereby release def SOL Productions and staff from any liability with respect to damage or injury that my participating minor child or I may suffer during participation in physical activity except where the damage or injury is caused by this gross or wilful negligence of the staff within the scope of their duties.

TERMS & CONDITIONS

We do not offer refunds or credits for recreational classes/workshop for any circumstances. If you miss a class due to illness or otherwise, we do not offer make up classes or credits. Classes are transferable to another student only if the student meets the criteria. We do not allow spectators. We require a minimum of 6-7 full semester participants to run a class. If low enrolment occurs, registered students will be notified at least 24hrs in advance and given a full refund or the option to switch to another class if available.

In the event that the government regulations require our host rental facilities (Saskatoon Salsa Dance Company or Second Act Studios) to cease in person operations, all def SOL classes will be moved online via Zoom. No refunds or credits given.

MEDIA RELEASE & COPYRIGHT

I hereby grant full permission to def SOL Productions to use any photographs, motion pictures, videotapes, recordings or any other record of my participating in dance, or any other related studio activities for any purpose including, without limitation, commercial use.

I understand that All Choreography/Video is the intellectual property of def SOL &/or the Guest Choreographers and may not be used, performed or posted online without written permission.

I hereby affirm that I am eighteen (18) years of age or older, I have read this document, and I understand its contents. I acknowledge that I am signing this waiver voluntarily and that if I am under the age of eighteen (18) I require parental consent.

I HAVE READ, UNDERSTOOD AND AGREE TO THE LIABILITY WAIVER, TERMS & CONDITIONS AND MEDIEA RELEASE WRITTEN ABOVE. I AM AWARE THAT BY SIGNING THIS AGREEMENT, I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE, WHICH I, OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS AND ASSIGNEES, MAY HAVE AGAINST THE ORGANIZERS AND RELEASES.*
