LIABILITY RELEASE: As parent/legal guardian representing the above-named child, I agree to release the Otsego County Sportsplex and Alpine Regional Tennis Association, its officers, employees, contractors and volunteers from any and all liabilityfor accidents, injuries, loss of, and/or damage to my/our person or property that may arise out of my/our participation in, or my/our presence at Summer Tennis activities.

I give permission for my child’s image to appear and be named in photographs and/or videos, in newspaper articles about the Summer Tennis programs, and any other story publications in connection with the Summer Tennis programs, including the Sportsplex/ARTA Facebook pages and/or websites.
