Signature* This field is required.By signing below, I am indicating that I understand the following conditions: 1) Funding is distributed by request of the grantee after the grantee submits documentation, such as receipts, invoices, or purchase orders, for expenses. 2) Funding can go towards an individual with a receipt to reimburse for pre-paid expenses or directly to a vendor with an invoice or purchase order. Pre-paid expenses can not pre-date the start of the grant cycle (August 1). 3) Funding can only be used for expenses outlined in the submitted budget. 4) Grantees have until July 31 of the following year to request funds. 5) Unused funds not requested by July 31 do not rollover into the next grant cycle without an approved grant extension. 6) At the conclusion of your project, all grantees will be expected to submit a short grant evaluation form and submit at least two digital photographs by July 31.
