    By submitting this form, I certify that I am requesting money for entrepreneurial use only. I certify that everything listed in this form is correct to the best of my abilities. I understand that I am allowing the owner of this form to contact me as well as any listed references to attest to my credibility. I understand and accept that this appeal, and future appeals can be denied if I am untruthful or appear to have been untruthful. I acknowledge that the owner of Unified Microfinancing can give a counter offer as to how much money to fund me depending on the circumstances. I understand that if my appeal is accepted I am required to follow up with Unified Microfinancing on my progress and send in testimonies and/or photographs of my undertakings within one month. I understand failure to do so with any of these will result in a ban from future appeals. I understand that I am either required to return the funds back to Unified Microfinancing in full once I am able to do so, or pay the money back in installments, therefore helping this company amplify and scale their mission, make it a self-sustaining entity in the long run of helping aspiring entrepreneurs.
