I, the undersigned, understand and agree to the following:
1. I give my permission to the Fort Liberty Spouses Club to share images from my photo session on their social media. I agree to hold harmless the Fort Liberty Spouses Club and its members for any harm which may result from the use of mine, my significant other's and my child(ren)'s images.
2. Payment is required at time of booking.
3. No refunds will be given. The timeslot requested may be given to another person only after both parties have emailed the FLSC stating they wish to do so.
4. My name and email address will be shared with the photographer in order to receive my digital images. No other information will be shared.
5. The photographer may or may not require a liability/image release to be signed prior to the start of the session.
6. I shall hold harmless the Fort Liberty Spouses Club (local, regional, national, international, and corporation), its volunteers, employees, paid or unpaid representatives, and/or the providers of the meeting/activity location from any liability or responsiblity for an accident, illness, or injury that occurs during or as a result of this activity. I accept that the final responsiblity for my, my significant other's, and my child(ren)'s safety rests with me.
