Waiver & Consent
1. I confirm that I am the owner or authorized by the owner of the pet(s) described on the application.
2. I confirm that my pet(s) is current on all vaccinations required by Jolly Paws Cottage LLC, is current with monthly parasite preventative, and is free of fleas/ticks/lice. To achieve full vaccine protection potential, I agree to wait 12 days after vaccinations before boarding my pet(s). I confirm my pet(s) has not been ill with any known contagious viruses in the last 30 days.
3. I understand that while my pet(s) is fully vaccinated that vaccines are not guaranteed and there is still a risk my pet(s) may contract a contagious illness. I agree that should this occur, I am responsible for my pets care, medical attention and expense.
4. I have informed Jolly Paws Cottage LLC of my pets health conditions and will provide medications. If my pet needs medical attention for any reason, I authorize Jolly Paws Cottage LLC to obtain veterinary care from my veterinarian or the nearest available provider. I agree I am solely responsible for all medical expenses for my pet(s).
5. If I am unable to pick up my pet(s) on time for any reason, I authorize Jolly Paws Cottage LLC to release my pet to the alternate person(s) listed above or continue boarding with Jolly Paws Cottage LLC at my expense. I agree that unpaid or unpicked up pets will be treated under abandoned pet procedure.
6. I confirm that my pet(s) is social and has not harmed or shown threatening behaviors towards any person or other dog. I release Jolly Paws Cottage LLC from any and all liability should my dog injure another dog or person and accept medical and legal responsibility for my dogs actions.
7. I release Jolly Paws Cottage LLC, its employees, and owners from any and all liability which I or my pet(s) may suffer including but not limited to injury, sickness, damage, accident, or death resulting from participation in boarding, walks, or any service provided by Jolly Paws Cottage LLC.
8. This form is applicable to any and all services for any pets for the named client with Jolly Paws Cottage LLC.
