- For NAMES and ADDRESSES, please capitalize the first letter of each word. For example: Cruz Maria Santos or Tacloban City, NOT CrUz MaRia santos or TACLOBAN CITY.
- The "Save and Continue Later" feature is not enabled in this form. Make sure you have a stable internet connection that will allow you to complete the form in one sitting. Many of the questions included here are from DepEd's LEARNER ENROLLMENT AND SURVEY FORM, which is required of all students.
- It would help too if you have your child's personal and school information at hand. For example, TRANSFEREES should be ready with the School ID Number and Address of the last school they attended.
- Upon successfully submitting this form, check your PRIMARY EMAIL account (including the Spam Folder). A system-generated message should have been sent to you, advising you of the next steps to take to complete your child's enrollment.

AGREEMENTS

1. I hereby certify that we seek the enrollment of our child/ward (whose name appears in this Enrollment Form) in STEFTI for SY 2023-2024.

2. I hereby certify that the above information given are true and correct to the best of my knowledge and I allow the Department of Education to use my child’s/ward's details to create and/or update his/her learner profile in the Learner Information System. The information herein shall be treated as confidential in compliance with the Data Privacy Act of 2012.

3. I hereby allow STEFTI to use photos, videos, and other information of my child/ward for the purpose of promoting the school and its students’ achievements, activities and events, through marketing or advertising materials, such as website posts, social media posts, physical and electronic bulletin boards, brochures, publications and other media PROVIDED, that this shall not constitute a waiver of my or my child's/ward's right to withdraw our consent for the use of such data in the future.

4. I understand that our child/ward can best be helped through the effort and cooperation of both the school and the home. I hereby agree to maintain an open and respectful line of communication with the school where the general welfare of our child/ward is concerned, and to abide by the rules and policies of the school set forth in the STEFTI HANDBOOK FOR STUDENTS AND PARENTS, a copy of which shall be provided to each student.

5. I hereby agree to actively support the programs and activities organized by the STEFTI Home-School-Community Partnership (HSCP).

6. I certify that this Agreement is effective and binding for School Year 2023-2024, and that failure to comply with the provisions of this Agreement may be grounds for STEFTI to refuse enrollment to our child/ward in subsequent school years.
