  • This form serves as a request for Commonwealth Terrace Cooperative to extend a license to permit Residents to keep the animal described below in the resident's unit.  A new agreement is required to add or replace an animal. Rules and restrictions must be followed or this license may be terminated. 

    The following animals are allowed:

    • Service animals and assistance animals (must be registered with UMN Office of Disabilities Services)
    • Dogs
    • Cats
    • Caged Animals
    • Aquarium Animals

    Residents must comply with the approved policy below and applicable animal registration fee that apply to animals at Commonwealth Terrace Cooperative. Residents are responsible for the cost of repairs for any damage to the unit caused by your animal(s).

    View corresponding fees with pet license on the current year's fee schedule. All fees associated with the pet/animal license will appear in RentCafé.

    In order to terminate the license, this form must be resubmitted.

    CTC requires each registered animal to have an additional two listed caregivers. Refer to the CTC Animal License Agreement for complete policy details.
