If you need to take advantage of our product support, please read through this entire document and complete the required information. Complete, sign and return ALL pages to us via one of the three ways listed:

Once received, we will issue a RA# (return authorization number) for you to add to this form and to your shipping label & box. Please include ALL pages of this original worksheet form with your shipment. We recommend making a copy of this completed form for your records. If you have any questions, please contact us at 321-242-8410 and we will be happy to assist you.
*Return Authorization Numbers are required for product support. Shipments received before a return authorization number is issued will be refused.*
Product Support Policies
All products that are returned to FightLite Industries for support are evaluated to determine if the service is to be provided at a fee or at a no- charge basis. Upon receipt of your shipment, our Product Support team will inspect the product and evaluate the specified issue or concern listed on the completed worksheet form. Please observe the following:
- All products must be thoroughly cleaned prior to inspection and your product should be thoroughly cleaned PRIOR to shipping to FightLite Industries. If the product is shipped to FightLite Industries and has not been cleaned a $100.00 cleaning fee will be charged and FightLite Industries will contact you regarding payment for this cleaning service prior to cleaning. Please note that payment in full is required before any inspection will occur.
- Upon completion of inspection, the customer will be contacted with a quotation for the cost of service and/or repair; unless it is determined at FightLite Industries’s sole discretion that charges may be waived. FightLite Industries will not provide service or repair for any of its products that have been subjected to unauthorized modification from its original factory condition. If your product has been altered from its original factory condition, FightLite Industries will provide a quotation for the cost and time to return the product to its original condition PRIOR to commencing work or performing further evaluation. Unauthorized modification includes but is not limited to alteration of handguards, barrels, chambers, feed ramps, receivers, top covers, feed trays, bolt carriers and bolts.
- For Shrike 5.56 & ARES-15 MCR Upper Receiver Assembly related product support, it is recommended that you ship the COMPLETE upper receiver assembly in order to permit FightLite Industries to perform a thorough inspection and comprehensive evaluation of the product. Please make sure that you properly secure the bolt carrier within the receiver assembly prior to shipping. PLEASE NOTE THAT THE OP-ROD IS UNDER HEAVY SPRING FORCE. PLEASE DO NOT PACKAGE THE UNIT WITH THE OP-ROD EXPOSED AS THE OP-ROD AND OP-ROD SPRING MAY BECOME A PROJECTILE.
- Safety Components: FightLite Industries regards certain parts as “safety components”. Under no circumstances will FightLite Industries knowingly release or return safety components that have been determined to be damaged, modified or found to be outside of their respective service limits irrespective of any individual’s or entity’s claim of right or ownership. In cases where safety components are found to be damaged, altered or outside of their normal service limits; FightLite Industries will attempt to contact and collaborate with the customer so that the product may be returned within proper service limits.
*Please Note: For the purposes used herein; it is within FightLite Industries Systems, Inc.’s sole discretion as to what qualifies as “thoroughly cleaned prior to inspection”, “unauthorized modification”, “altered”, “modified from original factory condition and or specification”, “damaged”, and “found to be outside of their respective service limits”.
You will be contacted by an FightLite Industries Representative prior to any work being performed to discuss the findings and quote for the service and/or parts. No work will commence without your authorization, quote approval and payment in full. If additional work is required outside the scope of requested service, this will also be included in your quote.
