Cancellation or Rescheduling Form

All program cancellations or rescheduling requests must be submitted using our form. No phone or email requests will be accepted.

Program Rescheduling

All rescheduling requests are subject to availability.
Programs can only be rescheduled within the same program title for the same child.
At the time of processing, if no alternate program is available, you can decide to remain in the original program or cancel your child's spot in the program according to the policy listed below.

Program Cancellation

Notice of cancellations must be received in writing no less than two weeks prior to the program date for a 75% refund. After this time no refund will be issued.
MOSAC will process cancellations within 2 weeks after submitting your form.
If receiving a partial refund, it will be returned via the original payment method unless otherwise noted.
