Baby Dedication Form
Congratulations on your desire to dedicate your child to the Lord! We are excited to assist your family inthis very important event. Baby Dedication ceremonies are usually conducted on fifth Sunday’s.At the appropriate time in the worship service, one of the Pastors will invite parents, grandparents and/ orgodparents and the child being dedicated to come to the front of the sanctuary to begin the BabyDedication Ceremony. At the conclusion of the ceremony, an official Certificate of Baby Dedication willbe provided to the parents.Fill out the Baby Dedication Request form on next page and turn it into the church office at least onemonth prior to the requested date.
