The Case Consult Service is a resource for healthcare practioner who would like help addressing an ethical question or problem related to a patient’s care. 

Case consultations are not formal ethical consultations and are not intended to replace any ethical consultations that you may do in your healthcare setting. These consultations are not intended to be placed in the patient's chart but are useful when you want to talk through an issue with a trusted colleague.

Complete this form with enough details of your case so that we can direct it to the best person to answer your questions. Please do not share patient identifying markers. We will put you in touch with a colleague who has a background in ethics and is best suited to discuss your question.
