  • Foster Home Form and Contract

    You must be 18 years or older to become a foster home. Please read all parts throughly.

  • Is your entire household in agreement to foster a dog?*
  • Fostering comes with challenges, Paws Up Rescue will temperament test all dogs prior to sending to private foster homes, however, You/your family understand there are no guarantees and Paws Up Rescue is not responsible for damages or injuries to any persons or property.*
  • Paws Up Rescue will provide you a protocol and guide for your rescue dog. This must be followed in order to ensure a smooth transition from one placement to the next. Are you fully prepared to follow through with this?*
  • You understand and agree that it can take 6 months to over 1 year to find a forever home for a foster dog. We are looking for a minimum of 3 to 6 month foster commitment for a foster dog.*
  • You agree to contact Paws Up Rescue's foster home coordinator if you want to return the foster dog to rescue with the complete understanding it may take up to a week or more to find another foster home for the dog.*
  • You understand and agree you will be fully responsible for the day to day care and well being of the foster dog.*
  • You understand and agree Paws Up Rescue LLC retains all ownership of the foster dog and can remove the dog at any time without explanation? You are agreeing to surrender the dog to a Paws Up Rescue LLC team member when requested.*
  • You understand and agree you must keep the foster dog in a safe environment, in your home.*
  • You understand and agree to keep the foster dog confined to your property via fenced yard OR leash walking only, no cables, chains, runners or tie outs.*
  • You understand and agree to contact Paws Up Team member with any behavior, training and/or medical concerns PRIOR to going to the vet, unless it is a life-or-death situation.*
  • ALL vetting must be done at Red Oak Vet clinic and must be approved prior to taking the foster dog, unless emergency.*
  • You agree to contact Paws Up Rescue Team member immediately if the dog goes missing.*
  • You understand and agree to return all crates, dog food, toys, potty pads, supplies, supplements and anything sent/given to you for the care of the foster dog once the dog is adopted or you return the foster dog. If you continue to foster another dog, items specifically meant for that dog will be sent with your new foster dog.*

    By signing below, you understand and agree to the terms above. You also agree this is considered a legal contract and you may be held responsible for any expenses occured if legal action is necessary.
