1.     Consent

(Herein after ‘Parent’ means a parent guardian or carer who has parental responsibility for the participant judoka).

·        I, being the above-mentioned competitor’s Parent, and I hereby confirm that he/she has my authorisation to participate in this Northern Home Counties (BJA) run event.

·        I further confirm and warrant that, to the best of my knowledge and belief, the competitor is able to participate in the Judo event. In permitting the competitor to participate, I am aware that the event will be live streamed and there is no expectation of privacy in that regard and Northern Home Counties (BJA) will utilise such live stream in relation to activities of Judo and/or for applying for funds to support these purposes.

·        If a medical emergency should arise during the competitor’s participation in this event at a time when I am not personally present to be consulted regarding the competitor’s care, I hereby authorise the Competition Organiser and Chief Medical Officer on my behalf to take whatever measures are necessary to ensure that the competitor is provided with any emergency medical treatment, including hospitalisation, which they deem advisable in order to protect the competitor’s health and wellbeing.

2.     Declaration of Consent - Child Aged 16 or over

·        I give permission for my photograph to be used within the Northern Home Counties (BJA) for display purposes.

·        I give permission for my photograph to be used within other printed publications.

·        I give permission for my photograph to be used on the Northern Home Counties (BJA)’s website.

·        I give permission for my photograph to be used on the Northern Home Counties (BJA)’s social media pages.

·        I give permission for video of me to be used on the Northern Home Counties (BJA)’s website.

·        I give permission for video of me to be used on the Northern Home Counties (BJA)’s social media pages.

·        I give permission for video of me to be used for training or analysis purposes.

·        I give permission for my details (including full name, weight and age category, club name and Area/Home Nation) to be used in pre-event entry lists and in event and post event results, on Northern Home Counties (BJA)'s website and social media pages.

·        I give permission for video of my child to be used on the Northern Home Counties (BJA)’s social media pages.

·        I give permission for video of my child to be used for training or analysis purposes.

·        I confirm that I have read, or been made aware of, how these images or videos will be stored within Northern Home Counties (BJA).

·        I give permission for my details (including full name, weight and age category, club name and Area/Home Nation) to be used in pre-event entry lists and in event and post event results, on Northern Home Counties (BJA)'s website and social media pages.

4.     Email and Contact

·        Contact will be made using the details provided in this form submission. Contact regarding this event will include pre-event information regarding this event.

·        If you do not wish to be contacted regarding this event or future events, please select the option not to be contacted.

5.     Prohibited Activities

·        Participants, parents and guardians and spectators will not be permitted to take pictures in changing rooms, toilet areas and at the weigh in portion of the competition.

·        In addition, photographs taken for personal use are not to be posted on social media as this may adversely affect participants where there is a safeguarding reason for their images not to be displayed.

6.     Disclaimer and Agreement

·        There will be a dedicated media team at the event in addition to a live internet broadcast of the event (connection permitting). Those conducting the filming and or photography at the event will have undergone the necessary checks.

·        By entering the event, all participants and parents or guardians of participants are consenting to photography/filming which may be taken by the BJA or an official BJA contractor at the event and in that regard there is no expectation of privacy. You also accept that these photographs or video may be used in future BJA social media and publications or published by the contractors in order to promote the positive aspect of our sport.

Northern Home County Rules for this Event:

1. Players must have an up to date Judo licence.
2. Strictly White Judogi Only
3. There will only be Gold, Silver and 1 Bronze per kata
4. In the event of a draw the ones that have drawn will do that entire kata again.
5. A pair cannot change roles in the same kata in the same age group.
6. If a player gets injured and cannot continue, they are out of that contest.
7. If a player has a minor injury the kata will stop and when possible, start again from beginning. 
8. There will be three judges and they will be seated approximately 1 meter apart, and they will score each technique 0 to 10
9. As the Gonosen-no-kata can be done in different ways, the format for this event will be: Two sets of 6 attacks with 6 counters as laid down in the BJA Dan grade technical theory Exam. This will also include the correct Kodokan Nage-no-kata. Bowing procedure at start and finish with the in-between tidy up at the end of the first set.
