Please fill out this online form to receive your prize payment.

This form allows you to quit and return to finish filling out information once you complete the email question. If you quit, the form will send you an email with a link to your partial completion to allow you to finish the form. Please do not start a second form - as it will create additional record, which will confuse the situation and delay your prize distribution.

If you lose your copy of the email with a link to this form, email us, and we will have the system send you an email link.

Please remember that this information is to pay you and provide you with required tax information if needed. It is important for you to use your name as it appears on bank accounts or your tax filings.

Typically the fastest processing method (and the one we use for our tournament staff and most of our vendors) is ACH. ACH stands for Automated Clearing House. It is used to make a direct deposit to your banking account.

Other methods may require us to transfer funds to a different bank/account to make your payment, since not all banks provide all distribution services for free. This transfer can take 2-3 days to clear, slowing your payment.

United States Tax Regulations require that we provide specific information to the Internal Revenue Service (IRS) for distributions of $600 or more. Consequently, we cannot pay any prizes of $600 or more until we receive the below information from you.

If you file taxes in the United States, if you have earned a prize of $600 or more, or your prize will bring you to $600 or more for the year from Caveman Chess, LLC, you must fill out and provide to us IRS Form W-9.

What are your preferred methods of e-payment?* This field is required.We can pay via ACH transfers **Preferred** (This method typically pays the fastest. Other methods often require that we send money to another bank/account, which can take 2-5 business days.) PayPal Venmo Zelle Deluxe e-check (an online check that you print and treat as a normal printed check. Deluxe is the company that prints checks for most US banks, and they have engaged in this secure payment method.) We prefer ACH transfer because it reduces fees and provides the easiest accounting. We suggest that you use Zelle only if necessary. It has lower limits than some of the other choices, so if several people select it, it may take several days for us to pay you due to the limits, or prizes may have to be split up and paid over multiple days.
