This Termination Agreement ('Agreement') is entered into on {agreementDate} by and between Young Event Group LLC dba A Twist of Lime, the 'Bar Service', and {signatory}, the 'Host', collectively referred to as the 'Parties', for services pertaining to Event ID#{eventId} on {serviceDate}.
By clicking 'I agree', I, {signatory}, certify I'm authorized to perform this action and authorize Bar Service to cancel services for the details corresponding with Event ID#{eventId}, and affirm my understanding and acceptance of the Termination Agreement (Version TA-2026-10-INT1).
Upon submission of this form, services will be cancelled immediately and all deposits and payments previously made to Bar Service will be retained and any monies owed will be billed or charged to the card on file at Bar Service's discretion.
