I / We authorize any consumer and/or commercial credit reporting agency, business, or person to provide any such information regarding us or the above business as may be necessary for consideration for this application. I/we give permission to Westfield Funding, LLC and/or affiliated company or subsidiaries to email, phone, text, fax, mail or any other means of communication. This authorization will be valid for two years from the date below. A photocopy, computer image, or facsimile of this authorization will be as valid as the original. The affiliation between Auto Pet Treat Inc and GU Loans is strictly limited to the provision of financial services by GU Loans and does not imply a partnership or any other business affiliation between the two entities. I / We are authorized to sign a binding contract for the above company. Funding may be provided by a third-party finance company or bank. I also give permission for Westfield Funding, LLC and/or affiliated company or subsidiaries to give the vendor the information needed to finance the equipment. This may include but is not limited to the approval status and finance limits in order to get your financing closed in a seamless manner.
