Adoption Eligibility
We strongly recommend that you read our detailed adoption process at http://dcsir.org/adoptionfoster-forms/ before completing an application to ensure that you are eligible to adopt in our area and for the specific dog(s) you are interested in. Fill out the application as completely and honestly as possible. Please note that DC Shiba Inu Rescue (DCSIR) is comprised entirely of volunteers who work full-time jobs. We try to respond to each application as quickly as possible.
Acknowledgement
The information I have provided is correct and true to the best of my knowledge and I have not withheld any pertinent information. I understand that any misrepresentation of me or my family or any untruths of the information I have provided herein that are discovered at a later date will invalidate any adoption agreement and will give DC Shiba Inu Rescue the right to reclaim the dog without my permission and without refund of any adoption donation. Submittal of this form provides my agreement to this puppy adoption application.

How do you envision mealtimes: the first week? The first three months? With other dogs? With children in the home?

DC SIR used to spay/neuter every puppy upon arrival until a few years ago, based on new scientific evidence. We now use spay/neuter contracts. What are the issues with early spay/neuter? What are the options?

The FDA has placed warnings on traditional flea and tick medications that have been linked to seizures and neurological disorders. Many people reach for the nearest drug their vet prescribes without question. How will you manage flea, ticks, and heartworm?

DC SIR follows minimal vaccination protocol and titers to avoid the evidence-based issues with over-vaccination. Are you aware of issues with over-vaccinations? Do you know what titers are? Do you know what core vaccinations are for dogs?
