This agreement is made and entered into on {todaysDate} by and between Fulton County Animal Services (hereinafter referred to as "FCAS"), LifeLine Animal Project (hereinafter referred to as "LifeLine") and {firstamp} (hereinafter referred to as "Fosterer" The Fosterer agrees to, voluntarily and upon consent, temporarily care for a homeless animal(s) on behalf of LifeLine and to agree to the following regulations:

The Fosterer understands that the foster pet is only temporarily in his/her care and remains the property of FCAS and LifeLine. Any and all adoptions of the foster pet will be handled through FCAS and LifeLine. The Fosterer is encouraged to assist in this placement but cannot make the final decision. FCAS and LifeLine or designee can remove the foster pet from the Fosterer at any time and without any prior notice.*

FCAS is a licensed shelter by the state of Georgia Department of Agriculture (GDA). Because foster homes are an extension of the shelter. foster homes must pass GDA inspection. The Fosterer agrees to an initial and periodic inspection by FCAS, LifeLine. and/or GDA.*

The Fosterer will make no decisions or actions regarding the foster pet's medical care (including euthanasia when an animal is put to sleep to end their life) without prior approval from FCAS/LifeLine All well and sick visits will be scheduled at LifeLine. In the case of an emergency. Fosterer will make every effort to contact the Foster Coordinator or the Shelter before considering an emergency visit. FCAS and LifeLine will not reimburse for unapproved emergency vet visits.*

FCAS and LifeLine will continue to provide preventative care (including vaccines, dewormer, flea and heartworm preventative) and medication for some minor illnesses diagnosed and treated by a FCAS and LifeLine approved veterinarian. Fosterer may be responsible for any medications, treatments or office fees for major illnesses or injury arising while in the fosterer's care.*

The Fosterer shall provide the foster pet with humane care and maintain him/her in accordance with all current state, county, and municipal laws and ordinances while the animal resides with the Fosterer. The Fosterer agrees to provide the foster pet entrusted to his/her care with adequate food, water. shelter, affection, socialization and exercise as (s)he would his/her own pets.*

The Fosterer shall provide the foster pet with a fenced yard or other humane means of exercise. The foster pet shall not be kept constantly tethered or outdoors. Foster dogs shall be leashed at all times when outside an unfenced area. Foster cats shall be housed exclusively indoors.*

The Fosterer understands that FCAS and LifeLine will not reimburse for any property damage incurred due to foster pet's presence in the home.*

The foster pet shall reside at the Fosterer's address If the Fosterer cannot keep the foster pet, the Fosterer shall notify FCAS and LifeLine immediately. The foster pet shall not be sold, abandoned, relinquished to a shelter or to another rescue group other than FCAS and LifeLine Fosterer will not relinquish custody of foster pet to anyone other than FCAS and LifeLine. And no person under the age of 18 shall attend to or care for foster pet without direct parental supervision*

The Fosterer understands that FCAS, LifeLine, and its representatives have limited information regarding the temperament and habits of the foster pet. The Fosterer will take all reasonable precautions (through the use of crates, leashes, supervision, and common sense) to ensure that the foster pet does not run loose. become a nuisance, destroy property. injure or be injured by another animal, or injure or be injured by any person or persons.*

Fosterer understands the foster pet may be incubating infectious diseases unbeknownst to the FCAS and LifeLine Veterinarian (S)He also understands that FCAS and LifeLine will not provide medical care or treatment to Fosterer's personal pets should they contract any disease from the FCAS/LifeLine foster pet.*

Fosterer agrees to provide. or coordinate assistance to provide, marketing materials about their foster*

Fosterer agrees to be contacted by a FCAS and/or LifeLine representative via phone and/or email to communicate relevant information that can include, but not limited to, foster pet medical needs, adoption events, and updated program protocols.*

In the event that the Fosterer wishes to permanently adopt the foster pet, an Adoption Questionnaire must be filled out and approved by an Adoption Counselor, and, if approved. an Adoption Agreement must be signed and submitted along with the appropriate adoption fees.*

Fosterer understands that FCAS and LifeLine have disclosed all known information regarding the foster pet and understands that the information provided to them about the foster pet may have been received by FCAS and LifeLine from third parties and this information cannot be relied upon and that FCAS and LifeLine do not warrant the accuracy or correctness of such information. *

Fosterer understands that animals can be unpredictable, and that FCAS and LifeLine cannot anticipate or ensure the conduct of animals fostered from FCAS/LifeLine Fosterer acknowledges that FCAS and LifeLine have not made through their agents, volunteers, or employees, any representations or warranties regarding the condition, health, temperament, or conduct of the animal.*

Fosterer understands and acknowledges any known behaviors of the foster pet and confirms they have been counseled on this information prior to signing this agreement. Fosterer agrees to maintain behavior training protocols supplied by FCAS/LifeLine for the foster pet should it be deemed necessary by FCAS and/or LifeLine.*

Fosterer agrees that if they are fostering an unaltered pet from FCAS/LifeLine, they will not have any other unaltered pet within their residence, unless they have received prior approval from FCAS/LifeLine's foster staff.*

The Fosterer does hereby release, discharge. and hold harmless FCAS, LifeLine, and any person, firm or corporation charged or chargeable with liability, their heirs, administrators, executors, successors, and assigns, from any and all claims, damages, costs, expenses, loss of services, actions, and causes of actions, whether known or unknown, belonging to the said Fosterer due to any action or occurrence from date of this agreement arising out of or in connection with the care of the foster pet*

The Fosterer agrees to pay FCAS/LifeLine any and all expenses. including court costs and reasonable attorney fees, in enforcing the terms and provisions of this agreement.*

The fosterer agrees that, if at any time they become non-compliant with the protocols set forth by the foster agreement and/or LifeLine staff, including becoming non-communicative, that the pet ownership will be transferred to them. This will include all medical responsibility and liability of the pet.

In an effort to find your foster pet the best home quickly, we would like to put you in direct contact with the potential adopter. By consenting below,you agree for your email to be connected to our Adoption Inquiry software to reach out to prospective adopters directly about your foster pet. You are required to reach out to interested adopters within a reasonable period of time and schedule meet and greets with interested parties. Fosters are expected to follow an Adopter's Welcome protocol during this process. FCAS will not share any other personal information with the adopter, and FCAS will not share your personal information with parties outside of FCAS and LifeLine Animal Project's overall operations.*

To better equip our foster parents, FCAS/LifeLine utilizes Maddie's Pet Assistant, which is a free smartphone application that sends periodic surveys to capture a foster pet's health and behavior and to supply the foster parent with tips and resources on how to resolve. Maddie's Pet Assistant also alerts the foster parent when the foster pet is due for vaccines. You will receive an initial email with an access code and instructions on how to download the app. LifeLine will not share your personal information with parties outside of FCAS, LifeLine, and LifeLine Animal Project's overall operations*

(1) The agreement must be on file at the above Animal Shelter's Licensed address, and a copy of this agreement must be on file at the Foster Home/Animal Shelter Agent's address

(2) All animals will remain the property of the above licensed animal shelter until an outgoing animal transaction is completed.

(3) If the licensed animal shelter or the Foster Home/Animal Shelter Agent's possession as soon as possible.

(4) Upon request from the licensed animal shelter the Foster Home/Animal Shelter agent must release possession of the animal(s) to the licensed animal shelter that is the owner of the animal(s).

(5) The Foster Home/Animal Shelter Agent must have license holder approval prior to rescueing any animal.

(6) The Foster Home/Animal Shelter Agent agree to authorize animal shelter personnel performing the recommended pre-approval inspection done prior to becoming a foster home and the required periodice inspection twice yearly (one during the summer and one during the winder is reommended). The Foster Home/Animal Shelter Agent agrees to inspections by GDA personnel when necessary.

(7) The Foster Home/Animal Shelter Agent Agrees to comply with all Applicable laws and rules, including the Bird Dealers Licensing Act, O.C.G.A 4-10-1, et seq., the Georgia Animal Protection act, O.C.G.A. 4-11-1, et seq., the Bird Dealers Licensing Rules, 40-13-12, et seq., and the Animal Protection Rules, 40-13-13, et seq.
