Achievement Award Nomination
The City of Lancaster seeks to honor individuals who have made significant contributions to Lancaster's Hispanic community over the course of their lifetime or as an emerging leader. Awardees will be honored at the City of Lancaster’s Hispanic Heritage Month Celebration on Saturday, October 5th.

Please be sure to list activities that involve service to the Hispanic community. Describe the most notable and impactful accomplishments of the nominee. Include details, with general dates, of the most important lifetime achievements that have contributed in positive ways to the City of Lancaster's Hispanic community. Provide examples of leadership in chosen career and/or servant leadership in City of Lancaster area organization(s). List affiliations with professional organizations, social or civic organizations, and clubs (past and present), including roles and responsibilities, titles and accomplishments. It is preferred that dates are included.
