The School Based Health Center, located at Maine East, is contracted with Advocate Aurora Health, and can provide therapy services to students at Maine East, West, and South. Therapy provides a safe space for one to process emotions, develop coping skills, and make progress towards needed change for their overall well-being. Some common topics discussed, but not limited to are: stress, trauma, self-esteem, dysregulation of emotions and behavior, or relationship and social issues. Therapy sessions are typically forty-five minutes and will be conducted with a clinical therapist in a confidential setting. This form details policies and expectations regarding services.
Disclosure: The clinical therapist may not be available at all times of the day. If in crisis or in an emergency during these times, please call 911 immediately for support.
Consent to Treatment
Per Illinois Minor Consent Laws, at the age of 12 - children may access mental health services for up to eight sessions prior to needing parental consent. After the eighth session, parental consent must be obtained to continue receiving services.
I agree to receive therapy services provided by the School Based Health Center with the clinical therapist. Treatment will consist of an initial assessment following a constructed treatment plan based on my goals and needs. Despite the positive factors of therapy, I understand there is no guarantee in "curing" my mental health concerns and continuing external therapy services is recommended. The length of services may vary per my needs and time as a student within District 207. If I move outside District 207 or after the summer of my graduating year, I will no longer be able to receive services through the School Based Health Center. I will be given resources/referrals to help transition my care into the community, and I can continue services if I so choose to. I am aware I have the right to withdraw and refuse the continuation of services at any time.
Confidentiality Policy and Release of Information
The School Based Health Center follows HIPAA privacy policies which will protect my identifying information, as well as all discussions during session are considered private and confidential. I understand the clinical therapist is a mandated reporter and there are limitations within the policy that requires disclosure without my consent; including: reports of abuse towards a child, an elder, or a dependent individual; reports of wanting to harm self or others; or if mandated by the court. In the occurrence of referring to external providers, I will be required to complete a Release of Information form for the clinical therapist to discuss my treatment with an outside entity. It is my right to provide consent or refuse for the clinical therapist to contact another provider for referral needs, which can limit my treatment plan.
Scheduling and Cancellation Policy
To receive therapy services, I must make an appointment. Walk-ins are accepted; however, there may be a waitlist. If I decide to walk-in for services, there is no guarantee of obtaining a same-day appointment based on the current day's schedule and/or waitlist. Continuing sessions are scheduled during opening hours to avoid missing class, which is subject to change per crisis. Cancellation Policy: Please contact the clinic to reschedule or cancel the appointment at least one day prior, so it does not count as a no-show. After three no-call no-shows, you will be placed on a waitlist and will be scheduled once availability opens.
