Please be aware that the utilization of any of the listed products could potentially induce a skin reaction or cause skin lifting. Your signature on this document serves as an acknowledgement of your understanding and acceptance of these potential risks.
Your signature also confirms your agreement to abide by the terms of appointment scheduling. It is incumbent upon you to be present at the agreed time and date. Concurrently, a deposit of $42.50 will be required, which may be applied toward future services upon the successful completion of the training session.
In the event of no-show, please note that the aforementioned deposit of $42.50 will be forfeited without exception. It is your responsibility to contact our scheduling coordinator a minimum of 24 hours in advance if there any changes to your availability are anticipated.
