Thank you for joining Old City Park as part of the entertainment lineup for Candlelight. We are so excited to have you participate in our 50th Candlelight!
Before filling out the form below please review the following:

-     Entertainers receive complimentary access to Candlelight on the day of the performance and receive one guest ticket per entertainer.

-     For student groups; each performer receives complimentary access to Candlelight on the day of the performance plus one chaperone guest ticket per performer.

-     Groups are asked to provide a list of entertainers and guests prior to the performance. Those lists should be sent to LauraDuty@icloud.com.

-     Candlelight will be held rain or shine. In the event of inclement weather, Old City Park will strive to provide a location for your performance that is under shelter. Some performances may be cancelled should weather conditions be unsafe.

- Candlelight promotes activities and entertainment of Candlelight in the weeks leading up to the event and encourage you and your group to do the same.

Liability and Hold Harmless

By completing this agreement, you acknowledge and accept that you, your agents, representatives, employees, heirs and/or assignees shall hold harmless the Old City Park and its agents, officers, and members for any and all liability of any kind arising out of your participation in the Candlelight event including but not limited to loss or damage to merchandise, third party liability for bodily injury or temporary injury or property damage, or product liability. You are solely responsible for obtaining whatever insurance you deem necessary to protect against the hazards and liabilities mentioned above.
