The nominee must be able to skate unassisted and be dressed in full hockey equipment. Any information obtained from this form will be used only by the Charleswood Hawks Jr Hockey Club for contact and selection purposes. Permission to use the flag bearer's name, team, and photo for the Hawks gameday programs, welcome introduction, and website will be obtained at least one (1) week prior to the game.
