Cover Sheet

Please complete the cover sheet which includes identifying information. The cover sheet will not be included as a part of the actual selection process by the review committee.

This form is to be completed by a team leader, department head, or counselor.

Reference letters should contain education-focused observations of the applicant’s knowledge, education experience, etc.

The reference letter author and applicant should have a recent working relationship, i.e. within five years of the application date. The relationship between the reference letter author and the applicant should be clearly specified in the content of each reference letter.

Please complete this form in its entirety. Please do not include any identifying information as the applicants are scored anonymously.

Upon completion, an email will be sent to the educator alerting them that a reference letter has been submitted on their behalf. No additional information will be provided in that email.

Do not include any personal identifying information regarding the applicant as applications will be scored anonymously.

Inclusion of any identifiable information will lead to disqualification of application.

Review of proposals is anonymous.

NOTE:

Review and consideration for funding is a “blind review” by assigned Foundation committee members. At nowhere in the application process should the applicant's name be identified. If the applicant's name is inadvertently placed in application, application may be subject to disqualification and ineligible for review.
