PRIVATE CLASSES BOOKING FORM

Please, make your private classes selections below. We will do our best to accommodate your requests, however, if the spots you have requested are not available, we will contact you and offer alternative options. Please, see, that some Instructors require a COUPLE to be present during the lesson. If you are not sure if your partner will be attending with you, please refrain from booking those lessons, to make everyone's lives easier and keep Nazar's hair his natural color longer and not turn grey prematurely:-)

IMPORTANT. Please, keep in mind, that at the time of filling this form, only 10 private classes can be requested. If you would like more than 10 lessons total, please, type how many lessons you would like in addition to the original 10 and with which instructor in the WISH LIST FORM below this form. We usually have some amount of cancellations and changes before the event and some slots open up about a month before the event. 

SUPER IMPORTANT:-) Please, also keep in mind, that the payment for the Camp and for the private classes submitted, is due within 15 days after requesting your private classes. If we did not receive the payment for your private classes within that time frame, we will gently remind you. If we still did not receive it after the kind reminder, your private classes might be released and your times given to another dancer. Please, please, please, help us to stay organized and bring you the most amazing and inspiring experience by registering early and paying for your private classes in advance:-)

THE WISH LIST

If you would like to request more than 10 private classes, please, add below the lessons (beyond original 10) who you would like to have lessons with. For example: Marzena Stachura-2, David Hamilton- 3, Irina Kudryashova - 1. If there will be private lessons spots open closer to the event (probably in December or so) we will contact you and schedule the lessons from the WISH LIST in addition to your original 10.

January 2026 Pricing

The CAMP PASS - Includes at least 10 participation Seminars by the Camp Instructors, (more might be added), 3 supervised practice rounds (Friday, Saturday and Sunday), 3 after-rounds Socials (Friday, Saturday and Sunday), priority booking for private classes, refreshments during the day. 

The Camp Pass Fee for January 2025 is $595 per person.

All participants MUST fill out Registration Form. For Private classes availability, bookings and pricing, please, contact us directly at +1(239) 961-5945.  We gladly accept personal, as well as business checks, Zelle and Venmo payments. All the Camp Passes and Private Classes must be prepaid within 15 days of scheduling those. If the payment is not received after 15 days after scheduling the private classes, those private classes may be released and the slots will be assigned to another participant. Private classes are booked on a "first come-first served" basis and are confirmed only after payment in full for the Camp and all desired private classes.

CANCELLATION AND REFUND POLICY. PLEASE, READ CAREFULLY WE HAVE MADE FINE PRINT QUITE LARGE FOR YOU:-) Full refund in the original form of payment will be given for the Camp Pass and all booked Private classes, if the request is submitted before December 10th 2025. We understand, that there are unforeseen circumstances, that might occur, and  will gladly offer you a full credit for the Camp Pass only, if request is submitted after December 10th 2025. No refund or credit will be given for prepaid private classes, if cancelled after December 10th 2025 WE LOVE YOU ALL, BUT, SORRY, NO EXCEPTIONS:-)

 

Assumption of Risk, Waiver of Liability, and Indemnification Agreement

IMPORTANT: This is a legally binding document. It must not be altered in any way, or it will become void. Please read carefully and ensure you understand its contents before signing.

In consideration of being permitted to participate in the activities associated with the IN Dance inc. dba Florida Heat Dance Camp (the "Activity"), held at Dancelife Florida Dance Center        January 8-13 2026, I agree to the terms of this Assumption of Risk, Waiver of Liability, and Indemnification Agreement (this "Agreement"), and make the following representations:

Assumption of Risk

I understand that participation in this Activity involves dancing and that there are inherent risks, which may include, but are not limited to: cuts, bruises, blisters, broken bones, strains, sprains, falls, concussions, torn muscles, internal injuries, spinal injuries, injuries caused by other participants, physical contact, exhaustion, dehydration, emotional distress, equipment-related injuries, collisions with others or objects, cardiovascular or respiratory issues, strokes, fatigue, poor decision-making, inattention by participants, equipment misuse or malfunction, damage to personal items, communicable diseases, and other hazards - whether foreseeable or not.

I acknowledge this list is not exhaustive and in no way limits the scope of this Agreement. I voluntarily accept all risks associated with my participation, with full understanding of the potential dangers involved.

Waiver of Liability and Indemnification

I voluntarily assume all risk of personal injury (including paralysis or death) and property loss while participating in the Activity. I hereby knowingly and intentionally waive, release, and agree not to sue, and further agree to indemnify, hold harmless, and defend Florida Heat Dance Camp, its directors, officers, employees, agents, volunteers, successors, and assigns from any claims, damages, or liabilities - including those arising from injury, death, or property loss - even if caused by ordinary negligence or omission.

Health and Conduct Acknowledgment

I confirm that I am in good health and have no physical limitations that would prevent my safe participation in the Activity. I agree to use equipment responsibly and to report any damage or concerns to staff immediately. I understand participation may expose me to communicable diseases, including but not limited to COVID-19, and I voluntarily assume this risk. I certify that I have read and agree to follow all posted rules and any future instructions provided by Florida Heat Dance Camp staff or representatives.

In the event of any injury during the Activity, I agree to notify a staff member or event representative immediately.

Public Filming Notice

By entering the premise, you grant Florida Heat Dance Camp and its affiliated partners full rights to use your image, voice, likeness in any media format, including but not limited to

1) Photography, 2) Video Recordings, 3) Livestreams, 4) Social Media, 5) Websites and promotional materials. NO compensation or further approval will be provided. IMPORTANT: If you do not wish to be filmed or photographed, please refrain from entering the event spaces.

Binding Effect

I affirm that I am at least 18 years old and legally competent to sign this Agreement. This Agreement shall be binding upon me and my heirs, representatives, executors, administrators, successors, and assigns.

I acknowledge that I have read and fully understand the terms of this Agreement and that I sign it voluntarily and of my own free will.
