Use this disclosure form if you have reasonable grounds to suspect that an IACP employee, officer, director or volunteer has been involved in inappropriate behavior including but not limited to theft, drug abuse, violence or threatened violence, or criminal damage, fraud, corrupt conduct such as bribery, false accounting, malpractice, undeclared conflicts of interest, unethical behavior, unsafe work practices, or misconduct.
You will be protected from any form of retaliation as set out in IACP's Whistleblower Policy and your case will be treated confidentially and securely. We have a zero-tolerance policy against retaliation of any employee who reported in good faith.
