The nature and method of the Permanant Makeup and/or Tattoo procedure has been fully explained to me, The risks that can occur with the procedure as well as the possibility of complications during and following the procedure have also been explained.
I understand there may be a certain amount of discomfort or pain associated with the procedure and that other adverse side effects may occur include minor or temporary bleeding, bruising, redness or other discoloration and swelling.
I understand that fading or loss of pigment may occur. Infection in the area of the procedure may occur, however, if properly cared for, is rare.
I acknowledge that I have read through all the FAQ as well as Pre/Post Aftercare listed on the businesses website, my artist also went over the information with me and I am in agreement and understand what to expect prior, during and after the service.
I acknowledge that I am aware that results will vary from person to person, as well as the amount of time the tattoo/permanent makeup will last. I also am aware I may need at least one touch-up after the service to complete the service, and that depending on my skin type or texture I may need additional touch-ups to achieve desired darkness.
I acknowledge by signing below, that I have been given the full opportunity to ask any and all questions that I have about the procedure or aftercare. All of my questions have been answered to my full and total satisfaction. I specifically acknowledge that I have been advised of the fact and matters set below and I agree as follows:
I acknowledge that it is not reasonably possible to determine whether I might have an allergic reaction to any of the pigments, dyes, topical preparations, or processes used in the procedure. I agree to accept the risk that such a reaction is possible and agree to proceed.
I acknowledge that complications are always possible as a result of the permanent makeup / tattoo procedure, particularly in the event that post-procedural instructions are not followed. I have received a copy of the After Care instructions to take home with me and agree to follow the instructions given. I take full responsibility for any and all post treatment complications.
I acknowledge that all subsequent procedures after the first session will/may have an additional fee and it is at my own expense and that it has been explained to me prior to my service.
I acknowledge that my body is unique and the technician cannot predict how my skin may react as a result of the procedure.
Red heads, blondes, and fair skin may be red, swollen and pigment MAY not take. Additional procedures may be required to obtain desired results.
Results WILL appear softer as the treated area heals. The area's treated WILL NOT look as DEFINED or as BOLD as the 1st procedure.
Microblading results typically last 6-18 months and may vary depending on the skin type, skin care and lifestyle. ALL procedures typically require an initial appointment as well as a touch up appointment. Color boosts should be expected to be done every 6 months to 1 year to keep the color fresh.
Powder brow results typically last 1-3 years and may vary depending on the skin type, skin care and lifestyle. ALL procedures typically require an initial appointment as well as a touch up appointment. Color boosts should be expected to be done every 1-2 years to keep the color fresh.
Tiny Tattoo/Fine Line Tattoos are permanent tattoos and though they will fade as well over time and need touch ups, I am aware that I will be receiving a permanent tattoo and I would like to proceed.
I am aware certain factors may affect my pigment more so, including: Certain medications, skin type, smoking, drug and alcohol use, use of cosmetic skin care products and makeup in eyebrow area. Activities that cause sweating, eyebrow plucking, cosmetic surgery, injections, and other factors, including health conditions, etc.
If I have any condition that might affect the healing of my PMU or Tattoo I will advise my artist, and I have filled out the forms to the best of my ability.
I acknowledge am not pregnant or breastfeeding.
I acknowledge and understand that if I have oily/severely oily skin the pigment may heal/appear much softer and can look more solid due to over-production of oil glands. The pigment will more than likely fade quicker. I accept this risk and would like to proceed.
Frequent tanning and sun exposure WILL heal darker and fade the pigment faster. It is recommended to NOT have a tan/burn (30 days before/after) on your face or body at the time of your procedure.
I acknowledge and understand that pigment implanted on darker skin types (i.e Indian, African American, Pilipino, etc.) The pigment will appear softer and blend more with your own skins melanin and will not appear as bold or defined as on lighter skin types and the hair strokes will be less visible.
Alopecia clients - Due to the change in skin texture, pigments may heal more powdered.
I acknowledge that the procedure will result in a permanent change to my appearance and that no representation or promise has been made to me as to later change or removal of the PMU or tattoo.
I acknowledge that the typical healing period is 4-6 weeks, after which a subsequent visit will usually be required. The final result will be achieved 6-8 weeks after the first visit at which the touch-up will/can be performed.
I understand that future laser treatments or other skin altering procedures, such as plastic surgery, implants and or injections may alter and degrade my permanent makeup/tattoo. I further understand that such changes are not the fault of the technician. I further understand that such changes in my appearance may not be correctable through future permanent makeup procedures/tattoo.
I acknowledge that no guarantees have been made to me concerning the results of the procedure.
I understand that the professional recommendation is a natural look for any (PMU).
I acknowledge that the correcting and revision of any type of PMU, Tattoo or microblading, previously performed by a different provider(s) involves additional risks and skill due to the unknown factors. Such as but not limited to, brand, color, time, skin reaction, depth and others. The technician has no responsibility for the final outcome or complications.
I accept responsibility for approving the color, shape and position of the pigments that will be applied. I understand the actual color of the pigment may be modified slightly due to the tone and color of my skin as well as the fade amount of the ink.
I acknowledge that the obtaining of the permanent makeup procedure or tattoo is by my choice alone and I consent to the application of the procedure and accept the risks.
I acknowledge that if I have had tattoo removal prior, due to scar tissue the pigment may not retain. Further procedures may not be an option.
In the event of MRI scan, I agree to inform my physician of the iron oxide permanent cosmetics as some pulling and burning sensation (rare) may occur during the procedure.
I understand that if I do not abide by the STRICT aftercare, I can ruin my results. The aftercare is crucial for optimum pigment retention.
Permanant makeup and tattooing is an ART, not a SCIENCE. Client's results with PMU will vary and using a pencil or powder may still be needed. We have no control over your bodies healing process and each time a procedure is done. The pigment will have less retention due to scar tissue. Touch ups will not be done any sooner than the recommended time.
I understand there will be absolutely NO refunds after services have been performed.
I consent that I am over the age of 18, am not under the influence of any substance or alcohol.
I release and waive any current and future claims, demands, actions, causes of action, suits, costs, liabilities, and damages against my microblading provider and the salon/studio. Its employees, owners and directors, offices and trustees for the right of privacy and publicity violation and invasion. I agree to not hold my technician nor the salon/studio responsible for any undesirable or unachieved results, or any reactions, during or after procedure.
