1. Only the School Registrar or Principal may certify the Grades Form 2. Compute for the average of all interrelated subjects taken within the same school year. ex. State the grades in standard quantitative equivalent Non-numerical grades should be recorded in numerical values (ex. 85.00 88.50 5. General Average for Grade 11 will include ALL subjects taken by the student in Grade 11, not just in English, Math, and Science. 6. For schools with a basic education program that does not follow the Department of Education's K-12 program but is recognized by DepEd to be its equivalent. indicate the General Average of the student during the most recent completed Grade Level instead of that of Grade 11. 7. Affix the school seal or its equivalent for validation. 8.ForUSTET subjects that were not offered in Grade 11, kindly record their grades for that subject in Grades 7 to 10 and their average. 9.
