Family Show Fee:

$60.00 for first family member + $30.00 for every other family member auditioning + ($330.00* for 15 vouchers (tickets)) = $420 (for two family members)

There is a possibility All Shook Up will also have a school show on January 25th and/or 26th. This will be confirmed at a later date and will be communicated by the artsitic team.

ATTENDANCE: I acknowledge the importance of regular attendance at rehearsals to ensure the quality of the show. I understand that more than two unexcused absences from rehearsals may result in my dismissal from the show. I further understand that any role I may be given can be reassigned should I not commit the necessary effort to the project.*

Liability Waiver

Liability Waiver: I understand that I or my child/ward is a voluntary participant in Orangeville Music Theatre's production. I agree to release and discharge Orangeville Music Theatre and any other related proprietors (i.e. schools, churches, theatres, dance studios and other venues used for the purpose of rehearsals, promotion, or staging the show) from all claims, demands and actions for injuries sustained to myself or my child/ward and/or damage or destruction, loss or theft of property of mine or that of my child/ward. I agree to release and hold Orangeville Music Theatre and any other related proprietors harmless form any cause or action, claim or suit that may arise. I attest that my child's/ward's attendance and involvement in such activities is fully voluntary, that I choose to or allow my child/ward to participate at my or his/her own risk, and that I have read the foregoing terms and conditions of this document.*

Code of Conduct

Code of Conduct: Participants will show respect by speaking and acting in a polite manner and being sensitive to the needs and rights of others (i.e. verbal and non-verbal body language and gestures towards others will not include profanity or harassment such as bullying, name calling, teasing, sexual or racial remarks). Help to create a safe space for all participants in which there is no fear of injury (i.e. participants will dress appropriately for activities, stay within given boundaries and not participate in fighting or rough play). Strive for success in the program by taking responsibility for learning, being on time and prepared, and being attentive during each session . Be responsible for the care of personal belongings and respectful of the property of others.*

Photo/Video Release

Photo/Video Release: I hereby give permission for images of me, captured during auditions, rehearsals, and performances with Orangeville Music Theatre through video, photo and digital camera, to be used solely for the purposes of OMT promotional material and publications, and waive any rights of compensation or ownership thereto.
