This Application Form is to be used for both the In-person and the Online course.

Starting Date of Course: October 2024

Your application will be considered by the training tutor, Bee Helygen, who reserves the right to select intuitively those who will best benefit from the training at this time.

Please note that the course is taught in English, so if this isn’t your first language, you need the skills to be able to follow everything without difficulty.

Please answer the following questions with as much detail as possible so that we can get a sense of who you are. The Temple College of Avalon will hold all information given in total confidence.

Please note that you must disclose below whether you suffer or have previously suffered from any mental health issues and whether you are on any prescription medication that may alter your natural state of mind.

We cannot take responsibility for any issue arising from the training that may trigger or exacerbate your condition. (This training will contain topics that may be upsetting to trainees, the Temple cannot be held responsible for any issues that may occur.)

N.B. The Temple College of Avalon reserves the right to terminate your training if these questions have been answered deliberately incorrectly and to demand payment of all fees due for the whole course.

The Temple College of Avalon follows all enforceable health & safety practices currently in place for training courses.

Neither the Temple College of Avalon nor the tutor accept any liability for damages occurred during the training.

Students accept that all knowledge is given in good faith. Students understand that they are entering into training in a pioneer field and all such training is given in good faith to the highest known safety standards of known or perceived practices within this discipline. If required, students undertake their own individual practice insurance.

I confirm that I have read, understood and agree to abide by The Temple College of Avalon/Swynwraig Training Ltd. course booking terms, conditions and boundaries and that the above information I have given is true and correct. I hereby state that I, or any persons acting on my behalf, will not hold or claim against The Temple College of Avalon, or any of its tutors or estates for any damages assumed or otherwise for any conditions, be it emotional, mental, spiritual, physical or financial relating to my training with the Temple College of Avalon.
