Dear Parent/Guardian, please use this form to update to your contact information. For address changes, you must provide a new proof of address. If you moved out of the district, you must also submit an open enrollment application. Once you submit this form and any required files, we will review and make changes in PowerSchool. We will contact the guardian on file if we need more information.

Do you need to change your student's address?*
You must provide a proof of address to make the change. The document must show the name of the guardian, the new address, and a date within the past two months.
- Utility bill
- Lease/rental/purchase agreement
- Bank statement
- Government letter or statement
If you moved out of the district, you must also submit a Change of Address Open Enrollment Application.
- Check the Boundary Map to see if your address is in the district.
- Go to the Enrollment Center webpage to download the application.
We will not update your student’s address until you submit the required documents.

Exceptions are made for people using Safe at Home or experiencing homelessness. Please contact the Enrollment Center and your school's social worker if this applies to you.
