We understand that surrendering a pet can be a tough and emotional decision, and appreciate that you are doing what is best for your pet and your family. Before bringing in your pet, we would like you to answer some questions for us to help you better.
The questionnaire can take 25 to 45 minutes; however, you may save your progress and return to it later. Providing as much information as possible is essential to make sure we can meet your dog's needs.
Please fill out one form for each dog. If you a have a litter of puppies to surrender, please email us instead.
We will send you a copy of your completed form for your own records.
All pet surrenders at the Edmonton Humane Society are by appointment only, depending on our available space and resources. This way, we can give your dog the best care possible when they arrive. The waiting times can vary, depending on your dog's needs.
Please remember to bring proof of ownership for your dog. Alternatively, you can bring proof that you have the authority to surrender the dog on the owners behalf when you come in for your appointment.
If we are unable to admit your pet, we will try to suggest other options or put you on our waiting list.
After you finish these questions, our team will review your form and get in touch with you within 10 business days to discuss when we can admit your dog and the next steps.
PLEASE NOTE: We currently have a waitlist and cannot guarantee a timeline in which we could accept your pet into our care. We recommend also pursuing other options, such as rehoming.
