Form is to be completed and submitted by the school in the fall. Information is required in advance of the Ministry Representative visit. The form responses serve as talking points at the time of the visit. Completion early enables the rep to adequately review and properly prepare, making for a more effective, efficient onsite visit experience. Upon completion of the form and after selecting "Submit" you may close the form out. It is submitted automatically and the "Thank You" box is the acknowledgement that the form has been submitted and received.
