In consideration of my teen's participation in the 2023 RMR Teen Rally, hosted by the Denver Church of Christ, I agree to the following:

Mandatory Reporting:

I understand that the staff, volunteers and all those associated with the 2023 RMR Teen Rally are required by law to report any suspicion of sexual or physical abuse to the appropriate authorities.

Terms of Acceptance:

I understand that the Rally Directors reserve the right to dismiss any participant whose behavior / attitude is disruptive or detrimental to the welfare of any participant, the virtual Rally experience, or to the Albuquerque Church of Christ, Colorado Springs Christian Church, Denver Church of Christ, Fort Collins Christian Church, and Salt Lake City Christian Church.I understand that my child may not keep ANY medication on them while attending the 2023 RMR Teen Rally. All medication must be turned into the nurse once your child arrives. All medication must be in it's original bottles.I authorize the appropriate use of my child's photograph that is seen on a screen shot from an activity or is visible on a recording of a lesson, class session or discussion group. Photos may be made available to the Youth & Family Ministry families in the home churches involved in this rally.I, as a parent or guardian of the Teen Participant named above, do hereby give my permission for my teen to attend and participate in the activities associated with the 2023 RMR Teen Rally.

Waiver of Liability

I hereby release, waive, discharge the Albuquerque Church of Christ, Colorado Springs Christian Church, Denver Church of Christ, Fort Collins Christian Church, Salt Lake City Christian Church, its' board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by above mentioned Teen Participant, while participating in any activity during the 2023 RMR Teen Rally.

By signing below I acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; that I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.
