Waiver 

In consideration of being allowed to participate in the event or activity referenced above, I acknowledge, appreciate, and agree that:

1) The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3) I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the Releasees, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR

OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Refund & Cancellation Policy
Terms and conditions for a refund: 

1. If we are unable to field a team for an event/season, we will issue a full refund. 

2. If Perfect Game or the tournament registered for, cancels the event without any games played, we will issue a partial refund. which will subtract the jerseys, hats, shipping, and admin fee.

PRIVACY POLICY
We respect and are committed to protecting your privacy. We may collect personally identifiable information when you visit our site. We also automatically receive and record information on our server logs from your browser including your IP address, cookie information, and the page(s) you visited. We will not sell your personally identifiable information to anyone.

Photo and Video/Media Waiver: 

I hereby permit and authorize Oviedo Training Center and its agents, sponsors, and others acting on its behalf, to use, reproduce and/or publish photographs and/or video that may pertain to the registered child, including their image, likeness and/or voice, without compensation.

I understand that this material may be used in various publications, public relations or marketing releases, recruiting materials, or for other endeavors specifically related to the promotion of the sport. This material may also appear on the organization's website or social media platforms. Consequently, the organization may publish materials, use my child's name, photograph, and/or make reference to my child in any manner that the organization deems appropriate in order to promote or publicize itself and its endeavors in relation to growing the sport.

I acknowledge the above media release waiver. If I do not wish to have photos and/or videos of my child released, I am to reach out to the organization.

Medical Release Waiver

In consideration of Oviedo Training Center providing a youth sports program, the undersigned, individually, and as a parent or legal guardian, does hereby release, indemnify, and discharge Oviedo Training incorporated, its officers, directors, coaches, and appointees from any and all claims, liabilities, and damages related to bodily injury or sickness and property damage sustained by the above child resulting from his/her participation, practice, play, or travel to and from play or practice in the Diamond Knights Organization. Furthermore, I agree to advise my child's coach at the beginning of the season/event of any and all medical conditions that may affect my child's participation in the Bombers program. However, I am responsible to be at practices and games if his/her special condition requires it.
