MAINTENANCE CONCERN
I hereby give permission for maintenance to enter to make the necessary repairs. The area the repair is located in will be easily accessible and cleared of all personal items.
I hereby understand if I miss the scheduled maintenance time I scheduled I will be charged a call out fee.
